Access to Medical Reports Act information and declaration

This consent form gives your client information about the Access to Medical Reports Act (AMRA), and a declaration giving Royal London permission to access medical history.

Use this supplementary form if your client answered 'yes' to any health-related questions in sections 6 to 9 of their protection policy application form.

Declaration of health

Fill in this form if your client wants to make changes to an existing plan, reinstate a lapsed plan, or if there's been a delay between application and a new plan starting.

Underwrite Later guide and declaration form

Information about our Underwrite Later cover, including terms and conditions and an application form. It can be used by clients who are applying for a protection plan by post, or who have already applied for Protection online without selecting the Underwrite Later option.
