Protection forms

Find forms to help you apply for and manage protection products for your clients, including our full range of Protection plans and trusts.

You can submit application forms for our Personal Menu Plan, Business Menu Plan, Relevant Life Plan and Pegasus Whole of Life Plan using our protection online service, or return forms to us using the address shown on the document.

For trusts, forms can be printed out, filled in and signed by hand. You can also complete the PDF digitally and submit with an e-signature, if it is submitted alongside the appropriate digital certificate. Please return a copy of the form to us once signed for noting – the original should be kept safe alongside the plan documents and other papers.

Clients can also complete a signature-free trust online while applying for Personal Menu, Business Menu, Whole of Life and Relevant Life Plans.

Clients can use this form to let trustees of their discretionary trust know who they would like to receive the benefits from their plan.

This deed can be used for discretionary trusts where the trustees want to ensure benefits go to specific beneficiaries. From this point on, the benefits will be held for them absolutely and cannot be paid to anyone else.
