A definite diagnosis of carcinoma in situ of the urinary bladder supported by histological evidence.
For the above definition the following are not covered:
- non-invasive papillary carcinoma;
- stage Ta urinary bladder carcinoma; and
- all other forms of non-invasive carcinoma.
